I just stumbled upon my old post here and thought I would update.
It appears that the issues with new leaf growth were due to a case of the thrips. They must have been in the crown of the plant and munching on the new leaves when they came out. They caused quite a bit of damage to this plant and a second phal but couldn't be seen until they came out to feast on new flowers.
I believe I have successfully treated the epidemic with regular spraying of rubbing alcohol on the plant, blooms, leaf crown and all.
