Not that I am an expert by any means but from the last picture the plant still appears healthy. I am wondering if the bark size chosen may be to large. A smaller size with a mixture of perlite and charcoal is usually a good mix.

Wow funny that you are posting this! I had my very first orchid casualty this week (granted I haven't been doing it this long)!!! It was a transplanted phal I moved from Sphagnum to bark. It was a rescue orchid in tiny pot. I had the same experience as you, with the yellowing dropping leaves. They didn't get crinkly looking, they just changed color and dropped. Unfortunately they ALL dropped. When I checked the roots they were all dead as well. It happened over less than 2 wks. Roots were fine when I put it in bark.. Big lesson for me!
