I would say that this is H. flavescens although the shape of the lip is very unusual. My plants showed this deformity too, when cultivated too warm.

This genera isn't mystic. It is the section Sortylos. Some of the species share the same distribution range and maybe there are several hybrids which haven't been identified until now.

I'd say that more than a form bothers me a color of the lip, to be H.flavescens.

Thank you all for all your inputs.

No, it is not a first time bloomer, Martin. It also got the same kind of lip shape last blooming.

I don't know the origin of this plant, but the label says it is H. flavescens.

About the temperature, the temperature range for the last six months is 55-70F, so the lip shape has nothing to do the the "warm" temperature. However, the flower buds opened up like deform flower, then petals developed fully(within 2-3 days) after the flower bud opened. Maybe this one has some kind of genetic problem. Here is a photo of my other H. flavescens, which it is growing under the same condition.
