When i read your title i was thinking that leaves got rot, but fom pictures your plant not in worth shape. Thats good
To me it looks your phal is underwatered, thats why leaves are wrinkled and soft. The moss looks dry, but roots look ok.
It has a chance to revive!
Most plans came from stores in moss, it might me good medium for greenhouses but not for regular homes. So you can hear lots of advises to repot it immediately in the bark. I agree on this one.
It is better for the beginners to pot in clear plastic pot so you can see roots and media and decide when to water. When roots dry they are silverish color, after you water it they will turn green. So if they are still green you dont need to water. But also you have to see if bark is drybor not, sometimes the roots closer to surface will dry faster than ones inside so if you water based on surface ones you might overwater the one inside.
If you can't find clear container you can make it yourself from Tupperware by making holes in it, or from Chinese take out container. Be creative. or you can order it online, there are many online sellers.
Your plant will survive few days with out pot while you looking for it.
At this time you can soak roots with moss in water for some time (5-10min) and then remove all moss, try not break the roots.
At this point it will be ready to repot. if you are not ready to repot yet, put the plant back in its pot and just water few times a day until you ready to repot.
