Treat mishmensis the same as any other Phaius culture. Intermediate-Warm temperature, lots of water and feeding during growing season, little bit dryer resting, low light (same as for Phal. or strap leaf Paph.). mishmensis come from the moonsoon belt and lower elevation than flavus, you don't need cool resting period, but need to careful about not feeding too much once the growth is mature.
You can add small/medium bark to the mix and use corse sand or aquarium gravel (regular sand easily wash away when you water the plant). Also the plant typically bloom heaviest from fall to spring. Just keep it in low light, humid and it will not send out any new roots till the new growth emerge, just like any other orchids with pseudobulbs. any new bulbs might be smaller and don't expect to bloom the following season as the plants suffer a set back.
