This orchid has literally done NOTHING for the last year. It looks almost exactly how it did when I got it. It is the top piece with 2 keikis attached from the mother plant that has been rooted and has done well. Except that lately there is something growing from it. Could these be spikes or are they keikis off the keikis?

Yes, they are spikes. and yours is way ahead of mine.
You "might" have spider mites problem too. take care of the mites or you will lose the spikes.
