Default Pretty new phal is Infected. Help!

I got a new phal for a pittance. I think the shop owner knew it was infected, but didn't let on. I was taken by the four pretty yellow flowers and vibrant green foliage. When I got it home, the signs of infection jumped out at me so I isolated it. One of the lower leaves had a 1/8" perfectly circular hole edged in black. Another had black edges (margins?) as well as places where holes of other shapes were developing. The axles, where flowers have fallen off or been removed, are black. I removed the infected margins and/or leaves and used cinnamon on the axle areas and places where I cut leaves off, or trimmed margins. A week later there are small holes edged in black are appearing on the perimeters of a couple of the petals, and a few very small white deposits on the backs of a couple of the petals.

I think I've attached a couple of pictures of the problem areas. I'll really appreciate your help.

P.S. If you could identify my plant for me that would be great, in case I have to replace it.
