I am trying to find the species name of this orchid. It bloomed for me in 2009 but has not done so since, so I would like to find out more information about its care so I can get it to bloom again.

yep, im with roy.
After further research now that I have a species name, I find that you are both right. I did repot the plant last year so that could be a reason why it did not bloom last year as I find they don't like to be repotted. I am wondering if the heat of South Florida might also be an issue, what I found is this orchid likes humidity but tempertures between 50-80 degrees, well the humidiy is not an issue but my 90 plus tempertures during the summer might be a problem, will have to wait and see.

I found that bit of information after researching after I got the species name, it will remain in the pot it is in now for many more years, I have had a lot of new growth but they never bloomed, I probably messed with the roots too much when I repotted.
