Yea, shes aint enjoying that media much! The roots could very well have started rotting....
What you do first, is soak some ORCHID BARK-never soil, never ever- overnight in a bucket of water, add a few grains of potash if you want to. Then, in the morning, drain the water and let the bark dry out a bit-remove most of the sunken bits-these are older and retain water too well. Then, take the plant from the pot, and put it in the kitchen sink with some luke warm water in it. Again, add a bit of potash as it is a good fungicide! Gently remove the rotted pieces of the old media and anything still clinging to the roots. Get a pair of sterilised scissors or secateurs, and cut off the mushy and/or brown roots-these are of no use to the plant, but plenty of use to virus' and fungi. Then, leave to dry on a paper towel for an hour or so-any green roots should start to turn white. Now, hold the plant in the pot, with the bottom of the stem JUST below the level of the pot. Add a small handful of the prepared bark, and gently tap the pot on the side of the bench-this settles the bark between the roots, where if you poked the bark in there, it would damage to roots. Repeat till the pot is full
As for the leaf, that is normal-the plant has grown another and has used all the nutrients it can from the oldest leaf, so it terminates the leaf
