Is it before the pseudobulb matures? Is it there waiting as the new leaf splits open? Or do they grow in shortly afterward?

Sheaths always form with the pesudobulb, like the second scenario you mention (waiting as the new leaves split open). I have never had a Catt alliance orchid that formed sheaths after the pseudobulb had matured. Pseudobulbs without sheaths can flower also some Catts will bloom from dry sheaths, but from my observations at least they will never form a sheath once the pseudobulb has completely matured.

They do?I have never seen one on mine. I have yet to see anything exciting on my Bc apart from more leads -that go nowhere..
