I have had my woes with Paphiopedilum. The biggest problem is water damage...it gets in the crown and the flower spike is no more! While I'm no expert in these matters, it is a possibility. Sorry for your loss, but look at the bright side....the plant looks otherwise healthy and next year you'll be smarter and better and have a wonderful flowering plant.
