You can mix with water normally 1tsp per gallon(5ml per gallon). Shake well before use. If infection is getting serious, you may up 2tsp per gallon(10ml per gallon).

Spray the whole plant including underside of leaves & don't water on that day as this may further dilute the physan and will become ineffective. Repeat every three days or so for about a month or three consecutive weeks.

Forgot to tell you, use immediately after mix with water. I think 1 gallon is too much for you, you can do this way also 1ml to 1liter roughly.

Thats a strange looking Miltonia, looks to be something other than. The leaves dropping, to me, appear to be from a change in growing conditions & cold. How long have you had the plant??
