You can un-pot it as long as you do not pull all the medium away from the roots. Although, it is probably fine if you do not notice and shriveled PB. Lots of catts have roots that grow out of the medium.

Do roots staying on top of the medium ALWAYS indicate a problem below? This Catt is in sheath, I hate to disturb it.

Yep, that's totally normal. Roots spread by habit, not necessarily because there's a problem inside the pot.
You should see some of the ones here I haven't got to repotting yet....
