Time to repot that phal orchid. It is currently over-potted and appears to have root damage (probably root rot).
First step is to take it out of that old medium and huge pot. Wash the roots with pure water and remove any squishy or rotten roots. Then take another photo and let us take a look. If it has some good roots, you can repot it. If it doesn't, you can spag and bag it. Hopefully it will have some viable roots.
