Phal in trouble

I really should have phal culture down by now but it seems I can grow other genera much better. This phal I have had for over a year. I repotted in bark, hydroton, lava rock, and perlite in July. It sprouted new roots and leaves in June. However it is slowly becoming limp. I noticed a black spot underneath a leaf and some "bumps" on another. Also the new leaf has a hole in it. I sprayed with physan 20 which is why the media is wet. I don't see insects. Any thoughts on it?

To me your plant quiet healthy as I can see the top few leaves and the buried roots too , it could be lack of water , and the black spot just get clean sharp blade and cut.

Is it just the bottom leaf that is limp and wrinkly? The new leaf at the top looks good, and if it's firm and not wrinkly, I wouldn't worry too much. If the more leaves are limp and wrinkly, I would consider soaking the aerial roots and putting them in the pot.

Then after cutting, use cinnamon powder on the fresh wound.

Hi,
It looks to me that the leaf on the left in the 1st picture is somewhat dehydrated. I have quite a few phals growing in cork, and some in lava rock. They are very happy. I water them every day.

All of the leaves are limp. I'm hoping that it's just dehydration. I have been watering every five days with this heat that we have had but I am guessing this one needs it more often. I just got my humidity trays in so I'm hoping the added humidity will help this orchid recover.

If the roots are green they are good. I mist the air roots daily.

Yes, it looks like it has been staying a bit too dry. The media is not holding enough moisture between waterings. Is there any way you can increase the humidity around this orchid? If so, that would help.

I have increased waterings to twice a week from every 5 to 7 days. I had stopped putting my finger in the mix every other day, but will have to remember to do that as seasons change. This has been the hottest summer on record here with most days being in the high 90's and even 100. The temperature in that room has been about 80 to 85. My humidity trays came in last week so I have it sitting on one with a bunch of other orchids. I am hoping that the humidity level will increase. I am getting consistent readings of at least 50% humidity. In the meantime I have also been misting the aerial roots every day. I hope that I have caught this in time. It so far is the only one reacting this way to the heat so perhaps it just needs a bit more water. It is the only one that I have that is a mixed with doritis so perhaps that is it?
